THERAPIST DIRECTORY AGREEMENT


This Agreement is made between:


Expat Therapy Hub PLC, registered at Albion Cottages, Walberswick, Suffolk IP18 3UB (“ETH”); and

[Name], an independent therapist whose details are set out in the therapist application form submitted via the ETH platform (“THERAPIST”).


This Agreement consists of:


  • the Therapist Form,

  • these Terms,

  • Annex A (Directory Listing & Subscription Terms),

  • Annex B (General Provisions & Arbitration)
